What Florida Asks For: The Long-Term Care Medicaid Document Checklist

Incomplete paperwork is the most common reason applications are delayed or denied. This is what to start gathering, whether you work with us or not.

Tip: tick each item as you gather it. Nothing is saved or submitted — it’s just here to help you keep track.

Have us handle this instead

Personal & Identity

For the applicant, and for a spouse if married.

Income

Every source, for the applicant and spouse.

Assets & Financial

Florida reviews five years of history.

Medical & Care

What establishes the level of care needed.

A few notes before you start

Five years of statements is the standard request. Florida reviews financial history going back five years, so gather more than you think you need rather than less.
Every account, including closed ones. Accounts closed during the review period still have to be documented, and a missing one stalls the file.
Do not move or gift assets first. Transfers made before an application is planned properly can create a penalty period. Talk to someone before anything moves.
This list is a starting point, not the full requirement. Florida can request additional documentation based on your circumstances.
Some of these need notarizing. We can handle that through Anchor Notary Group, our sister company, so it is one less appointment to arrange.
